"html": "<!-- BEGIN -->\n<p>COLLECTIVE/MEMBERSHIP AGREEMENT OF</p>\n\n<p>The Bright Spot</p>\n\n<p>A CALIFORNIA COLLECTIVE OF MEDICAL MARIJUANA PATIENTS</p>\n\n<p>The California Health and Safety Code provides that qualified patients may collectively associate for greater access to, and distribution among collective members of, medical marijuana. This collective consists of qualified patients and primary caregivers who associate in order to lawfully and collectively cultivate, process, use, and safely provide marijuana to each other for medicinal purposes. Patient members shall only be admitted to this collective’s membership after they have: 1) obtained a valid doctor’s recommendation; 2) presented valid state issued identification to prove the patient’s identity, and 3) signed the following membership agreement stating the patient has read, understood and agreed to all Collective rules. Membership may continue only so long as the patient's recommendation has not expired and all rules of the (The Bright Spot) collective (hereinafter referred to as the "Collective"), are followed.</p>\n\n<p>Members of the Collective shall include all patient members regardless of their method of contribution. Members include, but are not limited to, the patient members who administer the Collective’s work; the patient members who grow the medicinal marijuana for the Collective’s medical use; the patient members who transport the medicinal marijuana for the Collective’s medical use; the patient members who process and package the medicinal marijuana for the Collective’s medical use; and patients who contribute, money, goods, or services to further the Collective’s lawful purpose and activities.</p>\n\n<p>Members of this Collective are informed and hereby acknowledge that the Collective may enter into valid agreements and undertake actions to associate with other collectives to allow the greater access of cannabis and other resources to the individual members of the Collective.</p>\n\n<p>All applicants for collective membership shall read the following statements and, if in agreement, initial where indicated</p>\n\n<p>I hereby declare that a medical doctor recommended or approved my use of medical marijuana. I further declare that I am a qualified patient or caregiver under CA H&S Code §11362.5, §11362.7. As per CA H&S Code §11362.51, I am legally able to use, possess, and cultivate cannabis for medical purposes. I understand that I am allowed to do so through safe and affordable access such as the type provided by the Collective. In doing so, I agree to sign and follow all the Collective’s rules and regulations regarding the Collective's services. I also agree to pay all personal out-of-pocket expenses and reasonable compensation for the Collective’s member services.</p>\n\n<p>I hereby verify that I am a California resident and my personal medical marijuana will not be taken out of the State of California. I further verify and agree that my medical marijuana shall not be shared, sold, bartered, traded, exchanged or delivered in any other means to any other person outside of the Collective.</p>\n\n<p>I further declare and understand that my contributions to the Collective for prescribed medicinal products are used by the Collective to ensure the continued operation of the Collective and that any said transaction in no way constitutes a commercial promotion or sale of any item.</p>\n\n<p>As a member, I hereby agree, appoint and designate the Collective and its representatives, as my true and lawful agents for the limited purpose of assisting me in obtaining my legally prescribed medicinal marijuana. I understand that this means the Collective will be required to purchase from Collective members, possess, transport and distribute my medication for me and I grant them the limited authority to do so.</p>\n\n<p>I agree to possess my original, or a true and correct copy, of my recommendation when I am on the property operated by the Collective. I understand that my failing to do so may result in refusal of services. I hereby agree to all future changes of these policies as the laws for safe access develop. I agree that any violation of the terms of this Agreement or any other collective rules are grounds for immediate termination of membership.</p>\n\n<p>I agree to provide the Collective with all changes in my contact information, diagnosis, or primary physician immediately.</p>\n\n<p>I further understand that nothing within this membership agreement is designed to give me, as an individual patient, claim to property owned or possessed by the Collective, or the corporate entity or entities charged with administering the affairs and operations of the Collective. I understand that all such rights and obligations are governed by the controlling agreements of the entities charged with the Collective's operations, and that nothing within this membership agreement is designed to confer any rights upon myself as an individual patient.</p>\n\n<p> </p>\n\n<p>For Caregiver Members:</p>\n\n<p>If applicable, I hereby declare that I am a qualified caregiver under CA H& S Code §11362.5, §11362.7, et seq., that in addition to providing medical marijuana to (a) qualified patient(s), I provide housing, health or safety to that/ those patient(s). I have provided copies the patient’s valid recommendations to the collective.</p>\n\n<p>Privacy Policy:</p>\n\n<p>We collect the following personal information about you in the context of our text messaging program: your name, mobile phone number, and texting preferences (opt-ins and opt-outs). We use this information to manage our text messaging program, including to send you text messages. We may also e-mail you for information pertaining to business or for our own marketing efforts. We do not sell your information to any 3rd party company. Your wireless carrier may collect data about your wireless device usage, and its practices are governed by its own policies. We take measures to protect your sensitive personal information but cannot guarantee its security in transit or storage.</p>\n\n<p>If you subscribe to any of our digital services, participate in our programs, or sign up for alerts or email newsletters, we may send you promotional emails about our other products and services to ask you about your experience.</p>\n\n<p>I hereby affirm that I read, understand and agree to the terms of the Collective Membership Agreement.</p>\n<!-- END -->",
